IncluzIT PRO and HOME are integrated into a single digital health and home assessment platform that help streamline care planning, support targeted home adaptations, and improve communication between healthcare providers, patients, and other stakeholders.

CareNav AI is a digital assistant that helps families navigate complex senior care decisions using AI. Leveraging natural language processing (NLP), machine learning (ML), and real-time data, it provides personalized care recommendations, adaptive care plans, and access to care services. Designed for both families and care agencies, it shortens care placement time significantly and supports better-informed decisions.

To improve access to oral health care in Canada.

With GAC’s support, the Union of Medical Care and Relief Organizations – International (UOSSM) will aim to increase access to primary healthcare services through mobile clinics and strengthened referral networks, increase screening and treatment of moderate and severe acute malnutrition among children under five, and increase access to mental health and psychosocial support (MHPSS), including for survivors of Gender Based Violence (GBV).

Project activities include: (1) receiving and registering new arrivals; (2) providing protection services; (3) providing shelter, and other essential non-food items; (4) providing adequate water, sanitation and health services, including in reproductive health; (5) providing camp coordination and site management services; (6) providing protection, including against sexual and gender-based violence, child protection and working with governments to register asylum seekers and refugees, and help issue identity documents; and (6) expanding opportunities for durable solutions for people of concern.

To support the federal government’s commitment to maintain a strong and effective publicly funded health care system through investments in emerging and demonstrated innovations in priority areas, such as palliative and end-of-life care, home and community care, mental health care and other federal, provincial-territorial and emerging priorities.

The project will implement advanced capabilities to the Vitall Intelligence platform to enable practitioners to remotely manage diseases within their patient cohort thereby enabling them to provide better care, reduce comorbidities, and prevent hospital visits and stays.

Carelynk is expanding its healthcare staffing platform to offer hospitals and long-term care homes a unified workforce management solution.

This net-zero project will enable us to meet the current and future demand for families by providing essential social infrastructure enabling equitable access to pediatric health care, and reducing the burdens faced by travelling families.
