Records from January 1, 2020, to January 26, 2026, regarding briefing notes, issue notes, situation reports, management summaries prepared for regional or national leadership such as statistical reports, tracking spreadsheets, dashboards, or datasets, emails, correspondence or summaries discussing such matters at a program, policy, or case-summary level pertaining management misconduct within the Greater Toronto Area Region. Specifically, all aggregated or anonymized data, across all relevant program areas, that would have investigated/addressed a complaint, either formally or informally, broken down by the year and the following: number of complaints, investigations or allegations received; number investigated; number substantiated, unsubstantiated, or informally resolved; type or category of misconduct alleged; classification/level of employee involved; and the nature of any corrective or disciplinary action taken.
