Interim Federal Health Program - List of providers

Hi, I would like to get the list of all IFHP providers along with their names, addresses and phone numbers in csv format. There is an existing PDF with this info, however, it is very difficult to search and filter.  I am a software developer so I thought maybe I'd develop something that makes searching and finding healthcare IFHP providers easier.  Thanks
