The Evaluation Grants are expected to: Develop relevant research capacity leading up to future TiC funding opportunities; Inform the implementation or adoption of transitions in care activities within the health system to optimize outcomes of patients experiencing transitions in care by generating high-quality, real-world evidence; Identify clear and actionable health system changes, relevant to transitions in care.
