English: The Transforming Health and Care Systems (THCS) initiative, established under Horizon Europe and co-funded by the European Commission, aims to align regional and national research strategies, promote excellence, and reinforce the competitiveness of European health and care systems, while fostering EU cooperation and enhance European collaboration with non-EU countries. In 2025, 34 funding organisations launched the Joint Transnational Call (JTC2025) to support collaborative, innovative research projects. The JTC2025 seeks to: - Strengthen primary and community health and care systems. - Provide policy and decision-makers with tools and knowledge to guide necessary transitions. - Promote financial, organizational, and practice-based service innovations. - Encourage interdisciplinary and transnational collaboration. Projects must include concrete plans for deployment and adoption beyond the project’s duration and setting. Subtopic 1. Strengthening the Primary and Community Care System Subtopic 2. Systemic Approaches to Modernising the Sector Projects should contribute to THCS’s operational, specific, and global objectives, delivering the following impacts: 1. Health Outcomes: • Reduced morbidity and mortality. • Improved chronic condition management. • Enhanced mental health and well-being. 2. System Efficiency and Effectiveness: • Lower healthcare costs. • Fewer hospital readmissions. • Streamlined care pathways. 3. Access and Equity: • Improved access to high-quality care for all, regardless of socioeconomic status, location, or cultural background. 4. Policy and Practice Influence: • Evidence-based influence on policy, regulation, and clinical practices. 5. People Empowerment and Self-Management: • Increased patient and community engagement. • Support for individuals to actively manage their health. 6. Knowledge Generation and Dissemination: • Creation of new knowledge and best practices. • Effective dissemination to relevant community members and groups. 7. Ecosystem Approach: • Multidisciplinary and intersectoral collaboration. • Integration of diverse expertise (e.g., healthcare, digital health, public health, economics, humanities, education, industry, non-profits, end-users). • Embedding projects into organizational strategies to enhance transformational impact. • Strong links to broader ecosystems and policy levels to maximize reach and impact. 8. Sustainability and Environmental Impact: • Long-term viability of solutions. • Promotion of environmentally friendly health practices. • Adaptation to climate change.
