Automation for Coach Efficiency (ACE) will enable healthcare organizations and professionals, using NexJ Connected Wellness, to provide more timely and effective care to a larger population of patients. NexJ Connected Wellness is a cloud-based patient-facing virtual care platform for population health management that improves the ability for patients with chronic disease to actively participate in managing their own health and wellness, with the support of their caregivers, advocates, and healthcare professionals. As part of the ACE project, new features will be added to NexJ Connected Wellness to improve the timeliness and quality of feedback that patients receive based on their progress toward achieving their health and wellness goals.
