The objective is to develop a P-P software based application aimed to reduce two friction points for patient care: 1) the manual recording and tracking of health care, treatment-based path, salient information and contact information to capture ones complete health and treatment journey and a secondly, to develop a novel solution to find and interact anonymously with patient peers having similar health issues (disease-specific) by providing a P-P platform for support and guidance throughout ones health treatment journey.
