Title:
Hospital to community patient integration
Agreement Number:
934456
Agreement Value:
$119,900.00
Agreement Date:
Oct 1, 2019 - Feb 28, 2020
Description:
This project is to link Maples' existing primary care platform to Maple's hospital platform. This project represents an opportunity to address a key data integration hurdle for patients, which is that their records are typically not shared between primary and tertiary care. Further to that, it opens up an opportunity for hospitals and health care providers that are looking for more holistic digital care tools and solutions to follow their inpatients after they have been discharged or to pull in primary care records for their hospital stay. This is a large technology challenge that has not yet been solved in the public system, as system integration is challenged due to data mapping and stringent privacy policies.
