Forms are omni-present in health care. They are used to collect detailed information about a prospective client. Then, a clinician has the time-consuming task of reviewing and interpreting the form contents. This project creates an advanced method to collect and score clinical attributes of prospective clients and adds algorithmic routing to transmit the information to the optimal health service that matches the client needs.
