Description:

IncluzIT PRO and HOME are integrated into a single digital health and home assessment platform that help streamline care planning, support targeted home adaptations, and improve communication between healthcare providers, patients, and other stakeholders. This project includes a discovery phase to assess the platform’s technical integrity, scalability, followed by a development phase to address gaps, enhance performance, and ensure privacy and security compliance. The work will also build in-house technical capacity to support ongoing product evolution and ensure platform readiness for clinical pilot testing.
