The Healthy Canadians and Communities Fund (HCCF) promotes healthy living for people in Canada facing health inequalities and higher risk of chronic diseases such as diabetes, cardiovascular disease, and cancer. It supports interventions targeting key behavioural risk factors, including physical inactivity, unhealthy eating, and tobacco use, while fostering supportive physical and social environments. HCCF prioritizes reducing inequalities among priority populations, encourages multi-sector collaboration, and invests in innovative approaches to prevent chronic disease and address underlying risk factors effectively across communities.
