Patient-Oriented Research (POR), which is foundational to evidence-informed health care, refers to a continuum of research that engages patients/communities, including People with Lived and Living Experience (PWLLE), as partners throughout the research process, focusses on patient/community-identified priorities, and improves patient/community outcomes.

The project will implement a comprehensive Indigenous relations strategy aimed at educating Indigenous communities on the prevalence of human trafficking and increasing their capacity to serve Indigenous communities and heightening awareness of the services available to Indigenous victims and survivors of crime.

IncluzIT PRO and HOME are integrated into a single digital health and home assessment platform that help streamline care planning, support targeted home adaptations, and improve communication between healthcare providers, patients, and other stakeholders.

CareNav AI is a digital assistant that helps families navigate complex senior care decisions using AI. Leveraging natural language processing (NLP), machine learning (ML), and real-time data, it provides personalized care recommendations, adaptive care plans, and access to care services. Designed for both families and care agencies, it shortens care placement time significantly and supports better-informed decisions.

The goal of this project is to reduce risk of drug poisoning and related harms among underhoused people who use drugs following an urgent healthcare interaction. It will provide targeted harm reduction outreach and service coordination by hiring a harm reduction outreach team (Project Manager, Peer Workers, Harm Reduction Workers) to increase local capacity to provide dedicated support to individuals who use drugs experiencing homelessness. It will also facilitate the hiring of a dedicated coordinator role who will define and map clear service pathways by improving communication between hospital discharge staff, paramedics, and the outreach team; organizing the dispatch and scheduling of outreach workers; collaborating with partners to map harm reduction services; integrating tracking tools and software to streamline coordination; and actively promoting the service among community partners.

With GAC’s support, the Union of Medical Care and Relief Organizations – International (UOSSM) will aim to increase access to primary healthcare services through mobile clinics and strengthened referral networks, increase screening and treatment of moderate and severe acute malnutrition among children under five, and increase access to mental health and psychosocial support (MHPSS), including for survivors of Gender Based Violence (GBV).
